How We Rank
Each hospital is ranked by its average markup above Medicare — the percentage difference between what the hospital charges and what Medicare pays for the same procedure. A hospital with 1,900% average markup charges roughly 19x what Medicare pays, while one with 2,300% charges 23x.
This data comes directly from each hospital's machine-readable file (MRF), which they are legally required to publish under the Hospital Price Transparency Rule (effective January 2021). Medicare rates come from the 2026 CMS Physician Fee Schedule.
Important context
- These are list prices — insured patients typically pay less after insurance negotiations
- Uninsured and out-of-network patients are most affected by high markups
- Lower markup doesn't always mean better care — use quality ratings alongside price
- We compare ~5,000+ procedure codes per hospital for the average
